Laboratory and instrumentation questions on the Water Treatment Plant Operator's Exam not only for the exam, but also for his day-to-day work tasks. The questions most often are not very numerous. However, there are basic questions that an operator should know, referred to on the operator's exam are in bold type along with other questions that are common to plant operations. When operators take samples for lab analysis, a Chain of Custody Report must be completed. The Chain of Custody Report includes the following: 1. Name and title of sample taker. 2. Name of the water system/facility doing the sample. 3. Sample site. 4. Date and time of sample. 5. Preservative added to sample. 6. Refrigerated/put on ice or not. 7. What type of water sampled. 8. Field analysis for disinfectant residual. 9. Note whether sample was dechlorinated or not. 10. Date and time sample given to lab. 11. Signature of lab person accepting sample.
