PERIPHERAL ARTERY DISEASE

Path: Occlusion in the arteries (segments extending from the aorta below the renal arteries to the popliteal artery) from arteriosclerosis and atherosclerosis
•Nicotine use, diet, HTN, sedentary lifestyle, diabetes, increasing age, genetics
Assessment:
Pain: intermittent claudication
Pulses: diminished or absent
Skin: cool, pallor, dry, shiny

PERIPHERAL ARTERY DISEASE

The Antiplatelet
Pentoxifylline: increases erythrocyte flexibility, lowers blood fibrinogen concentrations
•CNS effects, heart failure, MI

Cliostazol: phosphodiesterase III inhibitor, vasodilator inhibits platelet aggregation
•Tachycardia, palpitations, heart failure

Clopidogrel: ADP receptor blocker/antiplatelet aggregation (binding is irreversible)
•D/C 5 days before surgery, HA, bruising, rash

Management:
Promote tissue perfusion
•Encourage to keep extremities lower than heart
•Moderate activity
•Pain management
•Teaching: avoid trauma, stockings, crossing legs, avoid extreme cold

Surgical Management:
•Endarterectomy
