Hct

Men: 42-52%

Women: 37-47%

  • proportion of RBCs to plasma
  • falsely elevated when WBCs are elevated ("buffy coat")
  • falsely lowered with hemodilution (increased fluid in blood)

Hgb

Men: 14-18 gm/dL

Women: 12-16 gm/dL

  • major component of erythrocytes (RBCs)
  • combines loosely with O2 and CO2 for transportation in perfusion
  • abnormal Hbg: sicle cell anemia, thalassemias
  • low Hgb: indicates anemia
  • Hgb & Hct run parallel (Hct = 3X Hgb)

RBC (erythrocyte count)

Men: 4.5-6.2 million cells/uL

Women: 4.0-5.5 million cells/uL

  • formed in bone marrow
  • removed by liver, spleen and bone marrow
  • life span ~ 120 days
  • carry Hgb molecules that carry O2 to tissues
  • abnormal values indicate anemia or blood dyscrasias; further evaluation would include testing RBC indices (MCV, MCH, MCHC)
  • RBC count lowers as we age
  • RBC counts below "normal" indicate anemia

RBC abnormal/decreased levels:

hemorrhage, hemoysis, anemia, hemoglobinopathy, advance cancer, bone marrow failure, leukemia, lymphoma, antineoplastic chemotherapy, chronic illness, renal disease, overhydration, multiple myeloma, pernicious anemia, rheumatoid disease, subacute endocarditis, pregnancy, dietary deficiency, prosthetic valves

RBC abnormal/increased levels:

High altitude, congenital heart disease, polycythemia vera, dehydration/hemoconcentration, cor pulmonale, pulmonary fibrosis, thalassemia trait, severe COPD

Platelets

150,000-450,000mm3

  • produced in bone marrow, 10 day life span
  • agglutinate to form plately plug in the clotting cascade
  • platelet stickness permanently inhibited by aspirin

< 150K indicates thrombocytopenia:

  • 20,000 or less indicates CRITICAL levels; spontaneous bleeding
  • immediately institute bleeding precautions
